As a reminder, if you're a General Agent planning to market Part D to Seniors, you must sign the Part D contract addendum and fax it back to Agent Licensing before your marketing efforts can begin. If you do not have this document, contact the Home Office.

Nov. 15 - Dec. 31
2008 First UA Medicare Part D enrollment forms may be submitted by fax at 469-525-4250.
